DONDE REFERIR NIÑOS PARA EVALUACION-WHERE TO REFER CHILDREN FOR DEVELOPMENTAL PROBLEM'S EVALUATION

Para niños de cero a 3 años  EN ILLINOIS lo mejor es comunicarse con FAMILY CONNECTIONS.  Busque el lugar mas cercano en internet en http://www.wiu.edu/ProviderConnections/links/CFCList.html
los servicios de evaluación y referencia de niños, y terapia si los niños lo necesitan son gratis. Por mi propia experiencia son personas maravillosas, y les estoy muy agradecido.(Angel Olmedo)

Para niños menores de 3 años de edad viviendo en Lake County, IL, puede llamar a Chid and Family Connections (847-377-8900). CFC cubre la totalidad de Lake County. Tienen personal bilingue.

 Si el padre vive en el Distrito Escolar de Waukegan (casi todas las direcciones de Waukegan se encuentran en el distrito de la escuela):

Edad de 6 semanas a 3 años, llame a Susan al 847-263-2319, Coordinadora de Educación Especial de Waukegan

Edad de 3 a 5 años de edad, llame a Rebecca Rios en el 847-249-6482, de Educación Especial de Waukegan (aquí hay una lista de espera, y puede tardar algunos meses antes que un niño pueda ser revisado)

Para inscribirse en la revisión de niños de 3 a 5 años, el padre debe ir a la escuela Waukegan District Welcome Center (en Greenwood, a pocos cuadras al este de la oficina de ChildServ). Rebeca Ríos es la persona de contacto en el Centro de Bienvenida. La oficina está abierta de 8 am a 4 pm Los padres deben traer la siguiente documentación:
1. Certificado de nacimiento
2. Dos comprobantes de domicilio (factura de la luz y el gas, tarjeta médica, contrato de arrendamiento, etc.)
3. Comprobante de ingresos (talón de pago reciente del padre o madre de familia que esté trabajando)
4. Los registros médicos, registro de vacunación
5. Todas las notas del médico o el maestro que se refieren a cuestiones relativas al desarrollo del niño.
 
            Si el padre vive en el Distrito Escolar de North Chicago:

Llame a Erica May (847-689-6333 x213) Departamento de Educación Especial del Norte de Chicago para programar una evaluación. Sólo para los niños de 3 a 5 años. Los exámenes se hacen sólo unas pocas veces al año (enero, mayo, tal vez de septiembre).

Ther evaluation, reference and therapy services are free. By my own experience, they are wanderfull people!
 
For any child under age 3 living in Lake County, IL, you can call Child and Family Connections (847-377-8900). CFC covers the whole of Lake County.
 
If the parent lives in the Waukegan School District (almost all Waukegan addresses are in the school district):
 
Age 6 weeks to 3 years, call Susan at 847-263-2319, Waukegan Special Education Coordinator
 
Age 3 to age 5, call Rebecca Rios at 847-249-6482, Waukegan Special Ed  (they maintain a waiting list; it may take months before a child can be screened)
 
To register for screening for Waukegan children age 3 to 5, the parent must go to the Waukegan School District Welcome Center (on Greenwood, just a few blocks east of the ChildServ office).  Rebecca Rios is the contact person at the Welcome Center.  The office is open 8 a.m. to 4 p.m.  Parents must bring the following documentation:
1.       Birth certificate
2.       2 proofs of residency (light and gas bill, medical card, apartment lease, etc)
3.       Proof of income (recent pay stub of any parent who is working)
4.       Medical records, immunization record
5.       Any doctor or teacher notes that refer to the child’s developmental issues that are of concern
 
If the parent lives in the North Chicago School District:
 
Call Erica May (847-689-6333 x213) at the North Chicago Special Education Department to schedule a screening.  They only screen children age 3 to 5 and the screenings are done only a few times a year (January, May, maybe September)
